The forms below are provided as helpful tools to support your group’s travel experience. While some may be necessary depending on your tour, others are here for your convenience and peace of mind. If you’re unsure whether a specific form is required, your Tour Advisor will know — feel free to reach out.
The standard set.
Parental Authorization Form
Required by some destinations for travelers under 18. Permission to travel with the group, signed by a parent/guardian no earlier than 30 days before departure.
Student Code of Conduct
Behavior expectations, travel rules, and disciplinary protocol. Signed by the student and a parent/guardian.
Travel Extension Form
For travelers who'd like to extend their trip beyond the group's return — meet the group abroad or fly home separately.
Limited Power of Attorney
Grants the Group Leader authorization to make on-tour decisions on behalf of a minor traveler when a parent isn't reachable.
Health & Medical Profile
Confidential medical history, allergies, medications, and emergency contacts. Used by Passports operations and Tour Directors.
UK ETA Information
Required for entry to or transit through the UK as of January 2025.
Parental Consent for Rooming
Specific authorization for rooming arrangements when a minor traveler is sharing accommodations with a roommate of the parent's choosing.
Activity waivers.
Required only on the itineraries that include the named activity. Your Tour Advisor will let you know which apply to your group.
