Required by some destinations for travelers under 18. Verifies the traveler has parental permission to travel with their group.

Important instructions

Read this first.

  • Your Group Leader determines whether the traveler must carry the Parental Authorization Form throughout the trip, or whether the Group Leader will collect them at the start of the tour.
  • We advise travelers to also have a photo of the signed form on their phones — handy if a paper copy is lost, and good practice at every immigration crossing.
  • Do not send this form to Passports Educational Travel. The form goes to your Group Leader, not to our office.
  • If you’re unsure whether this form is required for your tour, please contact your Group Leader.
Completion & documentation

What needs to happen.

  • Parents or guardians must complete the form and send a copy to their Group Leader no later than 30 days before departure.
  • Travelers must have a photo of the signed form on their phone.
  • Only one parent / guardian needs to fill out the form. The signing parent / guardian must also include a photocopy of their government-issued ID.
